Program Design
The program is designed in phases. As patients achieve the goals established for each phase, they gain confidence and a better understanding of how to effectively manage their heart’s health independently.
Phase I
This phase begins in the hospital soon after your heart problem occurs and ends when you leave the hospital.
At your doctor’s request, the Cardiac Rehabilitation team will visit you and your family to offer help and guidance as you slowly begin to increase your activity.
Phase I is complete when you are discharged from the hospital.
Allegiance Health considers this phase vital to your recovery. We provide it free of charge, since it is not covered by insurance plans.
Phase II
Phase II begins with your doctor’s referral after you leave the hospital to an outpatient program that includes both exercise and education.
You will complete Phase II in a group, with others who have heart disease. You will exercise on equipment while wearing a heart monitor, under the close watch of staff. We will keep your doctor informed of your progress while you are in the program.
The exercise portion is designated to help you regain strength and condition your heart in a safe setting. We will give you an effective treatment plan based on your own particular heart problem.
Education is also an important part of Phase II, and our classes will help you learn to live with heart disease. Our staff is available and willing to
answer your questions and concerns at any time.
We will work with you and your insurance company to determine your coverage for Phase II.
Phase III
Phase III is an ongoing fitness program for those who like to exercise in a group setting. A physician referral is required for entry into the program. Each person chooses an exercise plan using our equipment, and trained staff are present to assist you. Our focus is on preventing further problems and promoting healthy lifestyles.
Phase III is not covered by insurance, but costs are reasonable.
