We desire your experience with Allegiance Health to be as pleasant as possible—from start to finish. Whether you will be admitted into the hospital for inpatient care or are scheduled for an outpatient service, such as blood tests or x-rays, it is important we have up-to-date insurance and contact information.

After your appointment is scheduled, our staff will review and confirm your information. If anything is found to be incorrect or out-dated, we will make the necessary corrections when you arrive for your appointment.

Please bring these items to your appointment:

  • Photo identification
  • Physician’s order
