To ensure a smooth registration process, it is important we have up-to-date insurance and contact information, regardless if you will be admitted to the hospital or scheduled for an outpatient service.
After your appointment is scheduled, our staff will review and confirm your information. If anything is incorrect or outdated, we will make the necessary corrections when you arrive for your appointment.
These changes may also be made prior to your appointment through our Advanced Registration Department.
Please bring these items to your appointment:
- Insurance card
- Photo identification
- Physician’s order (if applicable, for lab work or certain outpatient procedures)
No appointment is necessary for a blood tests and x-rays. These services are available on a walk-in basis only.
