The Allegiance Senior Health Center accepts referrals from primary care physicians and social agencies. Services to meet the needs of elderly adults include:
Our staff will complete a comprehensive evaluation of the patient’s overall health and develop an individualized care plan.
Our geriatric team makes regularly scheduled visits to a number of skilled nursing facilities and assisted living facilities in the area. In some instances, it may be necessary to visit homebound patients in their homes.
Education & Resources
Educators on the Allegiance care team provide senior patients and their families with reliable health information relevant to their care. We also help identify resources and services available in the community.
Access to Special Health Services
To provide senior patients with the complete range of services they may need, the Allegiance Senior Health Center works closely with other areas of our health system, including areas, Physical Rehabilitation, the Hearing Center, Home Care, Hospice, and Home Medical Equipment.
